Purpose of the collaboration: This project aims to improve the quality, usability, and completeness of data captured during Voluntary Medical Male Circumcision (VMMC) service delivery in Gauteng through the revision of the Client Intake Form.
Description of the collaboration: Indlela, in collaboration with the National Department of Health (NDoH), co-designed and revised the VMMC Client Intake Form to improve its usability, accuracy, and alignment with routine clinical workflows. The revision process was grounded in behavioural science principles and informed by observational site visits conducted in Gauteng; provider consultations; and multiple iterative design cycles with Gauteng stakeholders.
The updated form incorporates several key improvements, including: simplified and streamlined content to reduce cognitive burden, clearer sectioning aligned to provider roles, and the introduction of visual signposting to guide completion and referrals. Additional enhancements include improved space for clinical notes, updated clinical content aligned with current guidelines, and better integration with the broader men’s health strategy. These changes were designed to support healthcare providers in completing the form more efficiently and consistently, while improving the quality and completeness of data captured during service delivery. Better and more complete data should translate to better quality service and improved client well-being.
The revised form has undergone eight iterations and has been approved by the NDoH for piloting in selected facilities in Gauteng under real service delivery conditions, where healthcare providers will be trained on the updated form and supported during implementation. During the pilot, the Indlela team will work closely with facility staff and district teams to: observe form use; gather feedback on usability and workflow integration; and identify areas for further refinement.
Insights from the pilot will inform final adjustments to the form and guide plans for broader scale-up across the VMMC programme
Expected outputs:
Sites: Improved quality and completeness of data captured through the VMMC client intake form in participating facilities. The project also aims to enhance provider efficiency and generate practical insights to inform further refinement and support broader scale-up of the revised form across the VMMC programme.

Target population: Healthcare providers involved in VMMC service delivery (including clinicians, nurses, counsellors, and data capturers) and male clients accessing VMMC services at participating facilities.
Duration: 15 months
